While completing the form does not guarantee assistance, it is vital for state and local agencies to gauge the scale of impact and coordinate their response effectively.

Officials estimate that the form takes approximately 15 minutes to complete. Residents are reminded to have information regarding their insurance policies on hand and are encouraged to document all damages with photos or videos.
