KANKAKEE, IL – Kankakee School-Based Health Clinic to host Back-to-School Clinic on July 10, 2023, offering physicals to Kankakee School District #111 students.
The clinic will provide school physicals and sports physicals to ensure students’ well-being. Appointments are required and can be scheduled at 815-802-5730. Walk-ins will not be accepted, and only 10 appointment slots are available. Patients must arrive 15 minutes before their scheduled appointment, complete forms beforehand, and bring their insurance cards or pay a $30 fee.
